Short answer
Take a clear, front-facing photo against a plain light wall, crop it to the size your form needs (35 x 45 mm is common in India), then print a sheet of copies at 100% size. The free passport size photo maker on this site crops, limits the file size and builds the print sheet in your browser, so your photo is never uploaded. Always check the exact rules of the office or form first.

Take a good photo
Stand facing the camera in even daylight, a step in front of a plain light wall. Keep your face level, eyes open, mouth closed and no shadows on your face or the wall. Ask someone to take it from about arm's length, at eye height.
Offices set their own rules for background, glasses, head covering and expression, so read them before you take the photo.
Crop to the right size
Common sizes are 35 x 45 mm for many passports and visas, 25 x 35 mm for a PAN card and 2 x 2 inch for US documents. Pick the size, drag the photo until your head is centred and your face fills most of the frame, then zoom in or out.

Online forms and file size
Many online forms ask for a small file, often under 50 KB or 100 KB, and sometimes a set pixel size. The tool can lower the file size to your limit while keeping the shape. If the form asks for exact pixels, resize with the image resizer first.
Print a sheet at home
Build a sheet of copies on 4 x 6 inch photo paper or A4, with thin cut lines. Print at 100% size, never fit to page, or the photos come out the wrong size. Measure one printed photo with a ruler before you cut them all.
Photo paper gives a better result than plain paper. A local print shop can also print the sheet file for a few rupees.
Keep it private
An ID photo is personal. This tool crops and prints in your browser, so the photo is never sent to a server. Delete the downloaded files when you are done.
